Our doctors incorporate Teladoc into their day-to-day practice as a way to provide people with convenient, affordable access to quality medical care. Does Teladoc replace my doctor? No. Teladoc doctors do not replace your primary care physician. Teladoc should be used when you need immediate care for non-emergent medical issues. It is an affordable, more convenient alternative to urgent care and ER visits. How do I set up my Teladoc account? Setting up your account is a quick and easy process. Simply visit www.teladoc.com and click Set Up Account. Follow the online instructions. How do I request a consultation to talk to a doctor? Log in to your account at www.teladoc.com and click Request a Consult. Or you can call 1-800-Teladoc any time day or night. How quickly can I talk to the doctor? A doctor will call you back within 30 minutes, on average. If you miss the doctor’s call (whether you are away from the phone or you have anonymous call blocker on) you will be returned to the bottom of the waiting list. The consultation request is cancelled if you miss three calls. Can I provide consultation information to my doctor? Yes. You have access to your portable electronic medical record at anytime. Download a copy from your online Teladoc account or call 1-800-Teladoc and ask to have your medical record mailed or faxed to you.

HOW TO REGISTER - Required: There is no fee to register on line. There is a one-time registration fee of $12 per member if registering over the phone prior to your first consultation will be charged.
- Required: Credit Card, Debit Card or ACH
- Optional: The name and telephone number of your pharmacy (if you do not have this information handy, there is a search tool on the registration page to help you locate a CVS location nearest you.)
REGISTRATION (Mandatory) Follow the steps below to register with Teladoc™. 1. Go to www.Teladoc.com and click Set Up Account. 2. Enter your legal first name, last name, date of birth and the company code. The company code is the group number listed on the front of your membership card. Note: You may also be asked to enter in your member ID in a future prompt so please keep your card available. After all have been entered, click Set Up Account Now. 3. Complete all required fields on the Account Set Up page. Under Login Information you may keep your generated username/member ID or you may change it now. *Please contact the Member Services number on your membership card if you experience login issues. 4. After you read the Terms and Conditions and Privacy Policy, signify your acceptance by entering your eSignature and then click Submit.
What's Next? Medical History Disclosure: Each Teladoc™ member must complete a Medical History Disclosure prior to requesting their first consultation. This is a separate step from registering your account and is similar to the paperwork you complete on your first visit to a doctor. You may complete your Medical History Disclosure online for FREE. If you do not have Internet access, call 800-835-2362 and a representative will take your medical history over the telephone. There is a $12 charge associated with this option and a credit card payment is required during the call. To complete your Medical History Disclosure online: - Logon to http://www.teladoc.com.
- Click on Login/Register in the upper right hand corner of the website page.
- Enter the username and password you created during registration.
- Complete the Medical History Disclosure.
- Complete the steps above for each participating family member.
When you need a medical consult, go online to request a consult or call 800-835-2362. Teladoc™ is not associated with any doctor network included in your benefits package nor do they have access to network information.
