You can use any of the services available to you in your area, as much and as often as you want, and discontinue at any time. With our easy to use discount card, you can benefit from our services whether you have insurance or not. We know that your family’s health is your number one priority. That’s why all your legal dependents can use your membership. If you wish to receive an additional card for a family member, simply email or call our member services department. For a small processing fee, they will gladly send out a card with that family member’s name. Please keep in mind that your membership is NOT insurance, so it’s simple and easy to use. There are no health restrictions or paperwork. You may use the card immediately and as many times as needed. When you present your membership card to a participating provider, you will receive a discount at the time of purchase. Please read the following instructions carefully in order to maximize your savings.

How to Use Your Membership:

  1. You can expect to receive your permanent membership packet in 15-20 business days after activation.
  2. Please read the description for each benefit.
  3. To locate participating providers in your area, check the listing included with this membership packet. For additional provider locations conveniently located near your home and work, simply call the appropriate number on the back of your card or make use of our online Provider Locator.
  4. Simply remove your membership card and keep it with you at all times so it will be easily accessible to present at provider locations.
  5. If you have any questions on how to use your membership, email us at support@procardinternational.com, and we will provide prompt friendly assistance.
  6. We offer a 30 Day Guaranteed No Risk Refund. If for any reason, you are not completely satisfied with our products or services, you may cancel and get a full 100% guaranteed credit refund of membership fees, excluding registration fee, if your cancellation is postmarked within 30 days of your activation.
  7. If for any reason you are dissatisfied with the services, you may terminate membership at any time by written notification to Procard International, LLC. Upon receipt of written notification to Procard International, LLC. membership will discontinue on the next one to two billing statements, depending on the processing date of written notification. There is no refund for processing or other membership fees after your 30 day no risk membership has lapsed. It is the mutual responsibility of the member and the company to keep the membership in force. To guarantee uninterrupted service, the company continues the services unless notified by member in writing. Acceptance of membership affirms this procedure.

Terms and Conditions of Your Membership:

Savings are based upon the provider’s usual and customary fees or national or regional average fees for the same service or product. Actual savings will vary depending upon your location and specific services or products purchased. The discounts contained herein may not be used in conjunction with any other discount plan or program. All listed or quoted prices are current prices by participating providers only and are subject to change without notice. From time to time certain providers may offer certain products or services to the general public at prices lower than the discounted prices available through this program. In such event, members will be charged the lower price. Providers are subject to change without notice and benefits may vary in some states. This is a discount membership program only, not insurance, and may be discontinued or modified at anytime. No portion of any provider’s fees will be reimbursed or otherwise paid by Universal Health Systems (UHS). Additionally, UHS does not guarantee, nor is it responsible for the quality of care or products provided by any provider.

California Terms and Conditions:

The California Terms & Conditions amends the UHS Agreement with Cardholders who reside in California, or Cardholders who purchase goods or services in California under the UHS Plan ("Plan"). Except as expressly amended, the Agreement remains unchanged. UHS administers activation in the Plan of Cardholders who desire discount pricing for goods and services, and vendors willing to reduce their prices to increase sales ("Participating Providers"). Cardholder hereby appoints UHS as Cardholder’s attorney-in-fact for the limited purpose of entering into an agreement with Participating Providers for discount pricing. The terms of a discount contract with a Participating Provider are incorporated into this Agreement.

California Cardholders hereby agree to pay for discount services and goods provided by Participating Providers who agree to accept a discounted amount for payment in accordance with the terms of the Agreement and these Terms & Conditions. UHS, or its appointed administrator, shall locate qualified Participating Providers and obtain their agreement to appoint UHS or its administrator as their agent for the limited purpose of entering an Agreement on their behalf with Cardholder to provide services or goods in return for discount pricing. Participating Provider shall thereupon be a direct party to the Agreement with California Cardholders.

UHS sole obligation under these Terms & Conditions and the Agreement is to administer contracts between Cardholder and Participating Provider (provide Cardholder identification cards, and a listing of participating providers with appropriate updates). Plan shall review discounted prices with a Participating Provider applicant and get the applicant’s approval of such pricing, and annually review such pricing to assure that it meets the reasonable discounting needs of Cardholders.

The Plan shall obtain the Participating Provider’s promise to:

  1. Accept a discounted fee from California Cardholders for goods or services, so long as the Participating Provider’s Agreement is in effect
  2. Comply with all applicable laws
  3. Be solely responsible to Cardholder for the quality of goods or services
  4. Promptly notify California Cardholder customers of withdrawal from the Plan
  5. Render any work in progress but not yet completed subject to the Plan, even if completed after the effective date of Participating Provider’s termination of participation in the Plan
  6. Cooperate in verifying the availability of a discount.
